• Other healthcare services as prescribed by physicians

Service Requirements

Physician Orders Required

All home health services must be ordered by a licensed physician. We work closely with your healthcare team to ensure appropriate care coordination.

Initial Assessment

A comprehensive assessment will be conducted by our licensed professionals to develop your personalized care plan. This assessment includes:

• Medical history review
• Current health status evaluation
• Home safety assessment
• Care goal establishment

Patient Rights and Responsibilities

Your Rights

As our patient, you have the right to:

• Receive quality care regardless of age, gender, race, religion, or ability to pay
• Be treated with dignity and respect
• Participate in developing your care plan
• Receive information about your condition and treatment
• Voice complaints without fear of discrimination or reprisal
• Privacy and confidentiality of your medical information

Your Responsibilities

As our patient, you agree to:

• Provide accurate and complete health information
• Follow the prescribed care plan

Payment and Insurance

Insurance Coverage

Service Limitations

• We do not provide emergency medical services - call 911 for emergencies
• Services are provided within our licensed service areas

• Background checked and drug tested
• Bonded and insured
• Committed to continuing education and quality improvement

Complaint Resolution

We are committed to addressing your concerns promptly and fairly. If you have a complaint:

2. Speak with our Administrator or Patient Care Coordinator
3. If not resolved, contact the California Department of Public Health
4. You may also contact Medicare (if applicable) at 1-800-MEDICARE

Liability and Disclaimers

While we strive to provide the highest quality care, we cannot guarantee specific medical outcomes. Our liability is limited to the scope of services we provide and does not extend to:

• Medical decisions made by your physician
• Actions taken against medical advice
• Emergency situations requiring immediate medical attention
• Conditions or complications beyond our professional control

Termination of Services

Services may be terminated by either party with appropriate notice under the following circumstances:

• Physician discontinues orders for home health services
• Patient goals are achieved or services are no longer needed
• Patient requests termination of services
• Safety concerns prevent safe delivery of care
• Non-payment of services (after appropriate notice)

Governing Law

These terms are governed by the laws of the State of California and federal healthcare regulations. Any disputes will be resolved in accordance with California state law and applicable federal regulations.
